Mafia: The Old Country (Mafia 4) is a third—person action story, a prequel to the series. The game takes place in Sicily in the 1900s, during the rise of the Italian mafia.

The main character, Enzo Favara, has worked in sulfur mines since childhood and wants to change his life. He joins the Torrisi crime family to become an established mafia boss. For the sake of a place in the sun, he has to do dirty jobs and errands. This is how the hero goes from an ordinary hard worker to a participant in criminal squabbles between various Cosa Nostra groups.

Features:
The linear structure of the narrative. There is no open world or side activities in the game.

The variability of the passage. Players can choose how to go through the locations: a frontal attack with exhausting shootouts or carefully sneaking up on the target and stealthily eliminating enemies.
